SPOILER for BOOK TWO : CLASH OF KINGS
Lets talk a little bit about some of the things that happen in Book two that HBO will just not do.
Here's a quick run down on elements to characters or plot that I think the HBOtv show will NOT cover:
1. At the end of book 2, Tyrion gets seriously jacked up, burned and loses a nose. I doubt HBO will have their lead character disfigured in in serious make-up all the time. I predict a small limp and cane, that will be it.
2. The Hound and Sansa. I doubt we will see on the show the last scene of the Hound, drunk at Sansa's chambers asking for that song and kiss.
3. Will we see Theon kill those farmer's kids that everyone thought were Bran and Rickton? Heads with tar on the pikes... if they were adults , maybe, but kids? I doubt it.
4. Will we see the Goat chopping feet and makeing stumps? This one we actually might see, not sure, there seems to be tons in Harrenhall that I bet will need to get cut down.

They've killed babies already so I'm pretty sure #3 will happen. Its one of the big shockers in this book so I doubt they would leave it out
I think they will show the goat torturing people to combine with the torturer in the village asking about valuables. It would save time and characters. And they totally will wound Tyrion's face but I think you are right in that they'll hold back on the nose.
in one of the trailers they show greyjoy men entering winterfell on horseback, on either side of the gate are boy sized bodies covered in tar, my guess is that they are the millers boys
